Hours Sussex 18:30/19:00-08:00 weekdays / 24 hours weekends and Bank Holidays Job Purpose To deliver high-quality, mobile urgent care to patients in their own homes across Sussex. The Home Visiting GP will provide face-to-face assessment and treatment for patients who are housebound, end-of-life or clinically triaged for a home visit following an NHS 111 disposition (CAT 3 or 4). Working with IC24 drivers and dispatch teams, the role ensures patients receive timely, safe and effective care within the community. Key Responsibilities and Accountabilities <ul><li> Undertake home visits for patients allocated through IC24 dispatch systems using CLEO on Toughbook devices. </li><li> Assess, diagnose, treat and prescribe appropriately within scope of practice and guidelines </li><li> Manage palliative and end-of-life symptoms in line with guidelines. </li><li> Prioritise visits based on clinical urgency and available resources. </li><li> Liaise with community nursing, pharmacy, paramedic and primary care teams to support continuity of care. </li><li> Document all consultations and handovers accurately and promptly. </li><li> Escalate safety concerns to on-call Service Manager and ICB as per escalation protocol. </li><li> Contribute to training, audit, and continuous improvement initiatives. </li></ul> Who are we? The Role Job Purpose To provide safe, timely, and effective clinical care within East Kent Urgent Treatment Centres (UTCs) based at WHH and QEQM. You will assess, diagnose, and treat patients presenting via NHS 111 bookings, A&amp;E redirections, and walk-ins, working collaboratively with nursing, reception, and allied professionals under the East Kent UTC Alliance (EKHUFT, Invicta Health CIC, Thanet Health CIC and IC24). Key Responsibilities and Accountabilities <ul><li>Deliver face-to-face consultations and urgent care for patients presenting with acute or minor illness and injury. </li><li>Undertake clinical assessments, investigations, and management plans in line with UTC clinical pathways. </li><li>Provide advice to patients and referrers on appropriate care, treatment, and safety-netting.</li><li>Liaise with A&amp;E teams, community services, and primary care partners to ensure seamless care. </li><li> Support effective triage, prioritisation, and redirection of patients where clinically appropriate. </li><li>Maintain accurate clinical records on CLEO and EMIS systems and complete statutory documentation promptly. </li><li>Work within IC24's prescribing formulary and clinical governance framework. </li><li>Participate in audit, learning events and educational sessions. </li><li>Promote team working and positive engagement with colleagues across Alliance sites. </li><li>Contribute to continuous service improvement and patient experience initiatives.</li></ul>Flexibility and Technical Requirements UTC GPs must be flexible to work rotational sessions including evenings, overnights and weekends on a 24/7 basis.
