Job Title: Occupational Therapist - Social Services Triage/Duty
Location: Aylesbury
Working Pattern: Full-Time (Part-Time considered)

What you will do:
* Conduct prompt triage assessments of incoming referrals to efficiently determine suitable support pathways.
* Undertake comprehensive holistic assessments of service users, primarily at their homes, to evaluate their functional abilities and environmental challenges.
* Recommend and facilitate major adaptations through the Disabled Facilities Grant (DFG) process, collaborating closely with service users, families, and stakeholders such as surveyors and housing providers.
* Assess and prescribe appropriate equipment to enhance safety, mobility, and independence, including complex moving and handling solutions.
* Collaborate effectively with multidisciplinary teams, including social workers, housing officers, and community health professionals, to deliver well-coordinated care and support plans.
* Provide clear and accurate documentation of assessments, decisions, and interventions in line with local authority procedures and legislative frameworks.
* Offer professional advice and guidance to colleagues, carers, and service users on matters related to occupational therapy.
* Support service users and their families through changes and adaptations that improve quality of life, ensuring a person-centred and strengths-based approach.
Requirements:
* A minimum of two years' post-qualification experience as an Occupational Therapist, preferably within a local authority or adult social care setting.
* Proven experience with manual handling, major adaptations, and processing Disabled Facilities Grants (DFG).
* HCPC registration as a practising Occupational Therapist is essential.
* Strong communication, assessment, and organisational skills.
* Ability to work independently and manage a varied caseload effectively.
* A valid UK driving licence and access to a vehicle is preferable due to the travel requirements of the role.
