What you will do:

* Assess patients in their homes who have been referred for occupational therapy intervention, tailoring care plans to suit individual needs.

* Perform remote assessments and expertly triage visits as necessary to optimise patient care.

* Work collaboratively with a multidisciplinary team to ensure comprehensive support for patients.

* Maintain accurate records and documentation to facilitate seamless communication and care continuity.

In addition to a minimum of two years of experience, you will need to be HCPC registered. While a driving licence is preferable, it's not mandatory. You will have the chance to partake in hybrid working, perfectly blending remote and on-site practices.
