What you will do:

* Provide advanced assessment and treatment for patients with complex needs, ensuring high-quality care.

* Offer expert advice, guidance, and information to patients, carers, relatives, and healthcare professionals.

* Manage the early assessment of patients referred from A&E, Same Day Emergency Care (SDEC), Enhanced Care Unit (ECU), and Acute Medical Unit (AMU).

* Collaborate closely with community services and partner agencies to maintain seamless continuity of service.

* Provide insightful expert advice within the multidisciplinary team.

* Supervise and support students, Band 5 staff, and assistant staff members, fostering a learning environment.

Requirements:

* HCPC Registration is essential.

* A degree or Graduate Diploma in Physiotherapy is essential.

* Car driver.

* Evidence of continuing professional development.

* Knowledge of relevant National Service Frameworks (NSFs) and national guidance.

* Proven ability in complex case management.

* Excellent communication and core IT skills.

* Evidence of supervisory experience.
