What you will do:

- Implement the Care Act 2014 and other relevant statutory frameworks to deliver high-quality care.

- Maintain a strong standard of professional practice, ensuring you are continuously developing your core skills and knowledge.

- Conduct specialist functional assessments to design and recommend interventions, effectively utilising resources.

- Prescribe preventative and specialist equipment along with minor home adaptations.

- Make informed recommendations for major home adaptations through the Disabled Facilities Grant process.

Requirements:

- HCPC registration is essential.

- A valid driving licence is a must to effectively provide services across the community.
