What you will do:

* Assess clients' needs within their living environments to develop personalised reablement plans.

* Collaborate with a multidisciplinary team to deliver tailored therapeutic interventions.

* Monitor and evaluate clients' progress, adjusting plans as necessary to achieve optimal outcomes.

* Provide guidance and support to clients' families and carers, fostering a positive environment for recovery.

* Keep detailed records and reports in alignment with professional standards and organisational policies.

Requirements:

* Applicants must be HCPC registered and hold at least 2 years of experience in Reablement services.

* A driving licence is preferable but not essential as travel to multiple sites in Southwark may be required.
