What You Will Do:

* Conduct community clinics and home visits around Greater Preston, Chorley, and South Ribble (PR1-PR26) to provide dietary advice and support.

* Implement nutrition support strategies, including home enteral feeding and basic diabetes management.

* Design and deliver personalised weight management programmes.

* Collaborate with multidisciplinary teams in community hospitals to enhance patient care.

* Educate patients and their families on maintaining a healthy diet and lifestyle.

What Experience is Required:

* A minimum of 2 years experience as a Community Dietitian.

* Home enteral feeding experience, nutrition support, diabetes management, and weight management are desirable.

* Must be HCPC registered to ensure the highest standards of practice.
