What you will do:
* Engage with the community by conducting functional and moving and handling assessments to determine the needs of both adults and children.

* Collaborate with the Home Improvement Agency to carry out joint assessments and help push forward referrals for Disabled Facility Grant applications.
* Work closely with company representatives to assess requirements for specialist or bespoke equipment, ensuring all needs are effectively met.
* Deliver clinical advice and recommendations to improve the overall healthcare process for clients in the community.
