What you will do:
* Focusing on the newly implemented discharge to assess process, you'll ensure smooth patient transitions from the hospital to home.
* Conduct bedside assessments at the hospital to evaluate patients' needs and readiness for discharge.
* Travel within North Oxfordshire, utilising your own transport, to complete home assessments aimed at promoting independence.
* Support individuals on their reablement journey, helping them regain confidence and autonomy.
* Carry out care act assessments, working towards tailored care plans that enhance patient outcomes.
* Attend occasional office meetings, remaining integrated with our dynamic team, while enjoying the flexibility of remote paperwork completion.
* To thrive in this role, you'll need to be at least two years post-qualified, HCPC registered, and a confident car driver.
