What you will do:

* Undertake client assessments using standardised assessment tools and the single assessment process

* Conduct holistic assessments and utilise standardised falls assessment tools to develop a multidisciplinary care plan

* Maintain effective multidisciplinary communications to ensure treatment goals are achieved and a high-quality service is delivered to clients

Requirements:

* HCPC registration is essential

* Minimum of 2 years relevant experience in physiotherapy

* A car driver is preferable to facilitate travel across multiple sites

* Strong communication and teamwork skills are a must
