What you will do:

- Conduct thorough assessments and implement treatment and discharge planning for complex and frail elderly patients

- Engage with the Discharge to Assess process, ensuring smooth and efficient transitions for patients

- Work primarily in inpatient settings, requiring hands-on involvement and a proactive approach

- Drive to access visits as needed, ensuring comprehensive care delivery

- Use RIO effectively, if experienced, to optimise patient care and treatment tracking
